Please include contact names (teacher, AP person), fax #'s, tel. #'s, AND PLEASE PROVIDE TEACHER'S EMAIL ADDRESS! Please let your purchasing agent/bookkeeper know that the order must be prepaid!   Please include the following on purchase orders: *Requesting teacher's name, shipping address, and, if possible, email and direct phone *Purchasing/accounting contact name, billing address, phone, fax. If you are sending a check WITH the order, be sure you have calculated the shipping cost.
